SLVHD Clinics
Many pharmacies across the Salt Lake Valley (Harmon's, Walgreens, Smith's) are now taking appointments and giving H1N1 vaccinations (ages 13+) Residents are encouraged to contact their private healthcare provider to check for H1N1 vaccine availability. Private providers continue to receive weekly shipments of vaccine.
How to Book Your Appointment Starting Tuesday (11/3) at 9:00 a.m. appointments can be booked: Online via Appointment-Plus Link will be posted on SLVHDs homepage at 9:00 a.m. SLVHDs Appointment Hotline at (801) 743-7280 Open from 9:00 a.m.-1:00 p.m., or until all time slots have been reserved.
Appointment Details Individuals who secure an appointment will be given a half hour time slot and clinic location for vaccination. All clients with an appointment will be screened at the clinic to confirm eligibility.
Eligibility Those eligible to receive the H1N1 vaccine:

- H1N1 vaccine clinics scheduled for Thursday & Friday this week.
- Individuals MUST have an appointment to receive their vaccination.
- Appointments are available in 15 minute increments, which is the average time it takes at the clinic with the new appointment system.

- Pregnant women
- Those who live with or care for children younger than 6 months of age
- Health care or emergency medical personnel with direct patient contact
- People age 6 months to 24 years of age
- Those under the age of 64 with chronic medical conditions associated with higher risk of medical problems from influenza.
