ORLANDO, Fla. — An employee performing a routine safety check Monday fell to his death off an attraction known as the world's tallest swing ride.

Jacob David Kaminsky, 21, was about halfway up the 450-foot StarFlyer ride in Orlando when he fell to the platform below.

The accident occurred at 7:40 a.m., before guests arrived at the ride, WKMG reports.

“It’s a shock and we’re saddened by it." said John Stine, Director of Marketing and Sales for the StarFlyer. "Anytime there is an injury or something of this nature, it’s extremely sad.”

According to records from the Florida Department of Agriculture, the ride had no prior history of safety violations.

The ride has been closed until further notice.
